Medicare Opt-Out Lookup: Has This Doctor Opted Out?

Search a doctor, therapist or other provider by name or NPI. You see whether they have opted out of Medicare, the dates, their Medicare enrollment and whether they can order and refer, plus exclusion checks.

Search by NPI, or choose Person and enter a last name and state. Each result shows the Medicare status panel with any opt-out.

Search the NPI Registry

Search by NPI number, name, organization, city, ZIP code or specialty. Every result is checked against the OIG, SAM.gov and state Medicaid exclusion lists.

Why the Opt-Out Status Matters

For patients: if your doctor has opted out, Medicare won’t pay for their services. You would sign a private contract and pay the doctor yourself. Checking before the visit avoids a surprise bill.

For practices and billers: claims for an opted-out provider’s own services won’t be paid by Medicare, while their orders for tests and equipment can still be covered (CMS). Staffing firms and credentialing teams track it when placing providers who see Medicare patients.

Where the Data Comes From

CMS publishes the Opt Out Affidavits file with each provider’s NPI, specialty and opt-out effective and end dates. We load the latest copy on every site update and show its date with each result. CMS notes that opt-out status renews every 2 years (CMS).

What This Lookup Can’t Do

  • It can’t tell whether a doctor is accepting new Medicare patients or accepts assignment. Ask the office.
  • A very recent opt-out or cancellation may not be in CMS’s monthly file yet.
  • Medicare Advantage plans have their own networks; check the plan’s directory too.

Medicare Opt-Out Questions

What does it mean when a doctor opts out of Medicare?

The doctor has filed an affidavit with Medicare saying they will not bill Medicare. Instead, Medicare patients sign a private contract and pay the doctor directly, and Medicare does not pay for those services, including any supplemental (Medigap) coverage tied to them.

How do I check if my doctor opted out of Medicare?

Search the doctor’s name or NPI above. The Medicare status panel shows “Opted out” with the start and end dates if CMS’s Opt Out Affidavits file lists them. You can also ask the practice directly before your visit.

How long does an opt-out last?

CMS says opt-out status renews every 2 years. It continues automatically unless the doctor cancels it before the end of a period.

Can an opted-out doctor still order tests or equipment?

Yes. CMS accepts opt-out as a status for ordering and certifying, so Medicare can still cover lab work, imaging, equipment or home health that an opted-out doctor orders, when the supplier bills Medicare.

Is a doctor not on the opt-out list sure to take Medicare?

No. Not being on the list means they haven’t opted out, but they may still not accept new Medicare patients, or may not accept assignment. Ask the office, and check enrollment in the same result.
