Why screen everyone, every month
- OIG’s own search takes 5 names at a time (OIG). A 60-person practice means 12 separate searches, every month, on OIG’s site alone.
- Monthly is the expected cadence. CMS told state Medicaid programs to require providers to search for excluded employees and contractors monthly (CMS SMDL #09-001). Massachusetts, for example, requires screening at hire and every month (MassHealth Bulletin 196).
- The penalty is per item or service. Billing federal health programs for work by someone you knew or should have known was excluded can cost up to $25,595 per item or service after HHS’s 2026 inflation adjustment, plus up to 3 times the amount claimed (HHS, 42 U.S.C. § 1320a-7a).
- Federal and state lists differ. A person can be on a state Medicaid list without being on OIG’s, and the reverse. That is why the check covers both.
How to prepare your list
- Export from payroll or HR as CSV, with first and last names. Add NPIs for clinicians if you have them: an NPI match is far stronger than a name.
- Add contractors, vendors and owners. Billing companies, staffing agencies, transport and supply vendors count too. Put a business on its own line, or in the business_name column.
- Include former names where you know them, on separate lines. People can be listed under a maiden or previous name.
- Check the count shown under the box before you run it, and use “Last, First” if your export lists surnames first.
What each result means
| Result | What it means | What to do |
|---|---|---|
| No match | No record with that name (or NPI) was found on any list checked, as of each list’s date. | Keep the PDF record with your compliance files and screen again next month. |
| Possible match | A record with the same name appears on at least one list. Many people share names, so this is not proof. | Compare the location, dates and job type, then confirm on the official source before acting. |
| NPI on a list | The NPI you entered appears on a list record. This is a much stronger signal than a name. | Confirm on the official source today and get advice before the person furnishes any more federally paid services. |
If someone is a possible match
- Open the row to see each matching record: list, dates, location and job type.
- Confirm identity on the official source: OIG’s search verifies with a Social Security number (OIG help); for SAM.gov and state lists, compare the identifiers and contact the agency.
- If confirmed, stop the person furnishing federally paid services, get legal advice on repayment and disclosure, and document what you did.
- If it’s a different person, note that on the PDF record and keep it on file.
For other states, use our directory of every state’s Medicaid exclusion list. For a single person or an NPI, use the OIG exclusion check or the NPI lookup.

How many names can I check at once?
Up to 500 per run, free. OIG’s own online search takes up to 5 names at a time.
Which lists does the bulk check use?
The OIG List of Excluded Individuals and Entities (monthly file), the SAM.gov public exclusions extract (daily file) and the California, New York, Pennsylvania, Arizona, Colorado, Georgia, Indiana, Iowa, Kentucky, Maryland, Missouri, Montana, Ohio, North Carolina, South Carolina, Washington state Medicaid exclusion lists. The PDF record names each list and the date of the copy that was checked.
Is my staff list uploaded anywhere?
No. The names are checked inside your browser against copies of the public lists. Nothing you paste or upload is sent to our servers.
What file format does it accept?
CSV or plain text. Use the template, or any CSV with columns named first_name, last_name, business_name, npi and state. Excel files need to be saved as CSV first.
How often should I screen my staff?
At hire and monthly. CMS told state Medicaid programs in 2009 to require providers to search for excluded employees and contractors monthly, and states such as Massachusetts require screening at hire and every month.
Who should be on the list?
Everyone whose work is paid for directly or indirectly by Medicare or Medicaid: clinicians, nurses and aides, but also billing, front desk and management staff, contractors, vendors and owners.
Is a possible match proof that someone is excluded?
No. Confirm on OIG’s search using the person’s Social Security number, on SAM.gov, or with the state agency that published the list, before taking any action.
Can it re-check everyone automatically every month?
Yes, with Practice Pro ($79 a month): save your list once and it is re-screened every month after OIG’s update, with an email when a new possible match appears and the dated PDF record every month.
